Imagine a world free from polio. Yet, in Karachi, Pakistan, a staggering **40,000 families are currently refusing polio drops, creating a significant hurdle in the global effort to eradicate this debilitating disease.** This resistance is particularly concerning because the polio virus has been detected in every district of this bustling city. It's a stark reminder of the challenges that remain in the final push to eliminate polio forever.
Pakistan has, in fact, built one of the most robust polio control systems globally. The country has achieved remarkable progress, slashing polio cases by over 99.6% since 1994. That's a huge drop from 20,000 cases annually to just 30 this year! The nation also boasts a massive surveillance network, with 127 environmental sampling sites and over 12,000 reporting locations, a testament to its commitment. Furthermore, the world has already eliminated two out of three global poliovirus strains.
But here's where it gets controversial... Despite these impressive achievements, Pakistan and Afghanistan remain the only countries where the virus continues to circulate. In 2024, the WPV1 strain spread across 90 districts, forcing authorities to implement a new “2-4-6 roadmap” under the National Emergency Action Plan.
Sindh Health Minister Dr. Azra Pechuho highlights the difficulty of reaching the finish line, stating, “The last mile is very hard. We are missing small pockets as the number of infected children drops.” This was shared during an event at Aga Khan University, attended by leading experts from the WHO and both federal and provincial governments.
Environmental samples confirm ongoing transmission in low-income and underserved areas, with every district in Sindh showing the presence of the virus.
Across Sindh, a total of 42,000 families rejected the vaccine this year, with 40,000 of those refusals happening in Karachi. Dr. Azra attributes this to “vaccination fatigue.” However, Dr. Sebastian Taylor from the Technical Advisory Group suggests that a lack of awareness, rather than vaccine overload, is a primary driver of these refusals.
The WHO maintains that progress is still being made. The number of children missed during campaigns has decreased from 1.48 million to 1.13 million. However, low routine immunization rates, vaccine hesitancy, and population movement continue to create opportunities for the virus to survive.
The most significant challenge lies in South Khyber Pakhtunkhwa. Prof. Shahnaz Ibrahim, Chair of the National Certification Committee, emphasizes the need for urgent and decisive action in this area. She also clarifies that Pakistan can only be certified polio-free after three years without any cases or environmental detection. This certification also hinges on Afghanistan's progress, as both countries are treated as a single unit for certification purposes.
Ayesha Farooq, the Prime Minister’s Focal Person for Polio, notes that the virus's genetic diversity is shrinking, which means fewer new strains are emerging. Dr. Azra adds that the upcoming low-transmission season will be crucial for the final push to eliminate the virus.
